I certify that I am eighteen years of age or older and that I am voluntarily requesting professional coaching services from Sensei Joseph S. Gracia, Ph.D., Doctor of Holistic Medicine, operating under Grace Ventures and Investments, Inc.

I understand that the services I am purchasing consist of a structured coaching program and not medical, psychological, or therapeutic treatment.

I understand and agree that I am enrolling in a twelve session private coaching program, with each session being thirty minutes in length.

These services are provided as professional coaching and personal development guidance and may include, but are not limited to:

• Life coaching
• Goal clarification and execution strategy
• Personal accountability and habit formation
• Leadership and performance development
• Emotional awareness and self regulation
• Lifestyle design and long term planning
• Application of the Twelve Pillars of the Sensei Joe Method

I acknowledge that this program does not include Reiki, energy healing, divination, or spiritual healing services.

All services are conducted virtually through digital communication platforms, including WhatsApp video or messaging.

I understand that this program is offered as a complete twelve session package and is paid in full prior to scheduling.

Once payment has been successfully completed, I will be contacted directly to coordinate and confirm my coaching session schedule.

I understand that sessions are scheduled based on mutual availability and that missed or unused sessions may not be refunded unless otherwise stated in writing.

I acknowledge that Sensei Joseph S. Gracia, Ph.D. is not a Medical Doctor, Psychologist, Psychiatrist, Licensed Therapist, Dietitian, Nutritionist, or other licensed health care provider.

I understand that he serves in the capacity of a professional coach and educator and that all guidance provided is intended for educational, motivational, and personal development purposes only.

I understand that coaching services are designed to support:

• clarity and direction
• disciplined execution
• accountability and consistency
• personal responsibility
• mindset development
• leadership and decision making

I understand that coaching is not a substitute for medical care, mental health treatment, psychotherapy, or legal or financial advice.

I agree to seek appropriate licensed professionals for any such needs.

I acknowledge that my results depend upon my own effort, choices, and actions.

I accept full responsibility for my participation, decisions, and outcomes during and after the coaching program.

I understand that no specific results are guaranteed.

By enrolling in or participating in this coaching program, I hereby release and hold harmless:

• Grace Ventures and Investments, Inc.

from any and all claims, demands, damages, or legal actions arising from my participation in the coaching services, to the fullest extent permitted by law.

I acknowledge that I am engaging in these services voluntarily and at my own discretion.

By completing my purchase and enrolling in the coaching program, I acknowledge that I have read this Agreement in full, understand its contents, and voluntarily consent to all terms and conditions outlined above.

I understand that my session schedule will be confirmed after payment is received in full.
